RELEASE, ASSUMPTION OF RISK AND INDEMNIFICATION AGREEMENT

FOR PARTICIPANTS - PLEASE READ CAREFULLY!

This Agreement for himself or herself, as First Party, participant for ministry, teaching, deliverance, healing in a group setting or personal setting, the undersigned, (   , are herein referred to as "Participant";

and

Journey2Wholeness Ministries any of its members known as: ordained ministers, board members, teachers, staff members, elder, pastors, deacons, team ministers, employees, officers, agents and volunteers that might be referred to as deliverance minister(s), teachers and/or team members that may assist him/her/them, as Second Party, are herein referred to as the (collectively) “J2W Ministry Team.”

WITNESSETH

PARTICIPANT UNDERSTANDS AND AGREES THAT THE J2W MINISTRY TEAM exercises freedom of religion, are engaged in the work of fulfilling the Gospel and call of Jesus Christ, carrying out the teachings of the Holy Bible, and are engaged in various ministries, including Deliverance Ministry, to those who may or may not be members of any church, who request or voluntarily submit themselves for recognized ministry by the J2W Ministry Team.

Participant understands that many of those engaged in the ministry to others are layman in the J2W Ministry Team, and without formal or academic training in the areas of ministry in which they are involved, but who seek to teach and to apply biblical principles, as they understand them through the Word of God, and leading of the Holy Spirit. Further, that no charge is made to those who seek and receive recognized ministry for the J2W Ministry Team; and based on the foregoing premises, the mutual agreements of the parties, and the good and valuable consideration, the sufficiency of which is hereby acknowledged,

Participant agrees as follows:

THIS IS A RELEASE ASSUMPTION OF RISK, COVENANT "NOT TO SUE" AND INDEMNIFICATION AGREEMENT. 
a. That the Participant acknowledges and agrees that the deliverance ministry has been explained to his or her satisfaction, and such ministry is sought and desired by the undersigned;
   
b. That the Participant freely, voluntarily, and without coercion, submits to the ministry provided by the “J2W Ministry Team”;
   
c. That the Participant understands that he or she may, at any time, terminate the ministry without any obligation to accept or to apply any part of the ????? same, or to be required to do' anything contrary to his or her free will; likewise the J2W Ministry Team is free at any time, to terminate the ministry without any responsibility or obligation to continue same;
   
d. That the undersigned is free at any time to consult any other person, church or organization for ministry or assistance.
   
e. That the J2W Ministry Team make no guarantees, or representation, either oral or written as to the result which may be obtained from this ministry;
   
f. That the Participant assumes all risk inherent in the ministry given by the J2W Ministry Team;
   
g. That no verbal or written representation has been made, or is authorized to be made to the Participant which is contrary to this written document;
   
h. That based upon the foregoing the Participant hereby release, discharge, and agree to indemnify and hold harmless the Parties of the J2W Ministry Team, it or their successors, heirs, administrators, executors and assigns from all loss, damage, cost, attorney fees, and any and every claim, demand, action or cause of action whether caused by negligence of the J2W Ministry Team or not, and whether now in existence or which may hereafter arise, as a result of, or in any way directly or indirectly related to, the ministry received from, the Parties of the J2W Ministry Team to offer or to continue such ministry. Further, Participant agrees and covenants not to sue "J2W Ministry Team” for any such claims, losses, damages, etc, as referred to herein.
   
i. The Participant agrees that this document agreement is binding on any and all of their heirs and representatives, and that the covenant "not to sue" is intended by all parties hereto to be as broad and inclusive as permitted by the Laws of the State of Texas and if any portion of the agreement is held, it is agreed that the remainder of this agreement, notwithstanding, shall continue in full force and effect.
   
j. The Participant understands the J2W Ministry Team is not a medical doctor or psychological counselor and we do not substitute for medical advice or treatment for specific diseases. We are ministers of the Gospel in accordance to His Word who heals and delivers.

READ CAREFULLY!

 I have carefully read this agreement. I understand it is a release of all claims, including the negligence of J2W Ministry Team.
   
I understand that I assume all risks inherent in the deliverance ministry set forth in the agreement and which I understand and voluntarily submit to.
   
I understand that I am making a covenant "not to sue" J2W Ministry Team.
   
I understand that I am agreeing to indemnify and hold harmless J2W Ministry Team.
   
I DO VOLUNTARILY SIGN MY NAME BELOW EVIDENCING MY UNDERSTANDING AND ACCEPTANCE OF THE PROVISIONS OF THE AGREEMENT.
